Semen analysis

If you wish to have a child and getting pregnant is not happening naturally, a semen analysis can be an accessible and important first step towards gaining insight into male fertility.

But what exactly does this test involve? What is tested? And what do the results mean for your chances of pregnancy?

Semen analysis

What do we assess?

  • Semen volume: The amount of semen per ejaculation. A normal value is between 1.5 and 5 ml.
  • Sperm concentration: The number of sperm cells per millilitre of semen. A healthy value is above 15 million per ml.
  • Motility: How well the sperm cells move. At least 40% of the sperm cells should have good motility.
  • Shape (morphology): The structure of the sperm cells. An abnormal shape can reduce the chance of fertilisation.
  • pH value: The normal pH value is between 7.2 and 8.0 and can say something about the health of the sperm.

Results and treatment options

What if the result is abnormal?

An abnormal result does not automatically mean that a pregnancy is impossible. Sometimes there are natural fluctuations and a second test may be needed. Depending on the cause, we may recommend treatments or lifestyle changes, such as:

  • Stopping smoking, limiting alcohol and eating healthily.
  • Avoiding extreme heat, such as saunas and hot baths.
  • Adjusting medication (if it affects fertility).
  • If further assessment or medical treatment is needed, a referral to another healthcare provider may be necessary. (Fertility treatment with sperm obtained through MESA, PESA or TESE is not possible at our clinics.)

What are the next steps?

If the semen analysis shows that the sperm is of good quality, further tests may be needed to find other causes of reduced fertility. If an abnormality is found in the semen analysis, the doctor will advise you on possible fertility treatments such as IUI, IVF or ICSI.

A semen analysis is an accessible and important test to gain more clarity about male fertility.
